With Co-Care management, you're in control of whom you see, when you see them and what they do. We give you the autonomy, tools and, most importantly, peace of mind that we've got you covered.
Our unique model offers you a dedicated Co-Care Manager who is available to support, guide and monitor your Co-Care management. PlanCare offers Registered Nurses to you where clinical expertise may be required or monitored.
With you taking the reigns coordinating your home care package, our team are working alongside you, providing the reassurance and support you may need.
PlanCare prides itself on providing you with a local service with national-level coverage. When you join PlanCare, you will be assigned your own dedicated Co-Care Manager, who will work with you to get the most from your HCP funding.
