How do I get a package reassessment?
It’s normal for care needs to change over time. If your situation shifts, a reassessment can help ensure your Support at Home package still provides the right support at the right time.


Sometimes the support you need can change and that’s completely normal. If your health, circumstances, or care needs shift over time, you may need to undergo a reassessment to ensure your Support at Home package still matches your situation. This process helps make sure you’re receiving the right level of support, at the right time.
If you are a transitioned client (i.e. you had a Home Care Package before 1st November 2025), you will receive a new Support at Home package when you undergo a reassessment.
Here’s how it works:
Request a reassessment
You or someone on your behalf (like a family member or provider) can contact My Aged Care to request a reassessment. This may be due to a change in your health, mobility, living situation, or care needs.
Assessment arranged
My Aged Care will organise for an assessor to visit you at home or contact you over the phone. This could be a Regional Assessment Service (RAS) assessor or an Aged Care Assessment Team (ACAT), depending on your situation.
Reviewing your needs
During your appointment, the assessor will talk with you about your current needs, any challenges you’re facing, and what’s changed since your last assessment. They may also review your current services and support plan.
Updated recommendations
After the appointment, the assessor will determine whether your level of funding or types of services should change. You may be moved to a higher funding level, or new supports (like Assistive Technology funding) may be added to your plan.
New support plan
If changes are approved, you and your provider will update your support plan and budget to reflect your new needs.
Reassessments are a normal part of making sure your care continues to meet your needs. It’s all about helping you stay safe, supported, and confident at home.
