Planning and coordinating your care to ensure services are tailored to your individual needs, preferences, and goals.
Regular reviews and updates to your care plan to reflect any changes in your circumstances, health, or priorities.
Monitoring service quality and provider performance to make sure you receive consistent, high-quality support.
Liaising with service providers and health professionals to resolve issues, and advocate on your behalf.
Providing guidance and support to help you navigate the aged care system and make informed decisions about your care.
This helps ensure your care plan remains safe, individualised and up to date.

Support at Home Classifications and Funding
The Support at Home program introduces eight funding classifications, ranging from ~$11,000 to $78,000 per year. Under Support at Home, budgets are set quarterly, with Care Management automatically deducted. Clients will have access to the full value of their budget at the start of the quarter. The quarters are as follows:

1 January - 31 March
Quarter 4
1 April - 30 June
If you are a new client who hasn’t received a Home Care Package previously, you will have a Support at Home package at one of eight levels. Your annual budget will be split into four even quarters, with 10% deducted automatically for Care Management.

If you are already receiving a Home Care Package, you won't need a reassessment. Under the Support at Home program, you will be moved across on your current Home Care Package funding amount, called a Transitional Home Care Package budget.

Support at Home Contributions
Under Support at Home, a client contribution refers to the amount a you may be asked to pay towards the cost of your care and services. The government will fund 100% of your clinical care costs, and a percentage of the cost of everyday living supports and services that help you maintain your independence at home. Here is a brief overview:

Income Assessment
Contribution percentages are set by Services Australia via an income and assets assessment. Completing an income and assets assessment isn’t mandatory. However, if you choose not to complete one, you can be asked to pay the maximum contribution rate. You can read more and start your application here.
