We’re a registered Support at Home Provider
From meal delivery and nursing to gardening, we connect you with the right support to suit your needs. Below is a list of PlanCare’s standard hourly rates for each service, effective from 1 November 2025.
To expand on the large range of services we provide, we can also partner with third parties for services such as gardening. If you would like a service not listed on our price list, talk to us and we can arrange the service for you.
Clinical Supports
These are health-related services delivered by qualified professionals and these services are fully paid for through your funding program.

Care Management fees
With PlanCare, you will have a dedicated and experienced Co-Care Partner to help you and be your key point of contact. Care management applies to all participants in the Support at Home program.
Under the Support at Home program, 10% of your quarterly budget is automatically paid directly to the registered provider (PlanCare) to provide Care Management activities. All Support at Home clients, regardless of the service they choose at PlanCare, will have 10% of their quarterly budget put aside for Care Management by the Government. Our Care Management fees are set out below. Fees will be charged in 15-minute increments, with a minimum in home service duration of one hour.

Care Management includes tasks, such as:
Planning and coordinating your care to ensure services are tailored to your individual needs, preferences, and goals.
Regular reviews and updates to your care plan to reflect any changes in your circumstances, health, or priorities.
Monitoring service quality and provider performance to make sure you receive consistent, high-quality support.
Liaising with service providers and health professionals to resolve issues, and advocate on your behalf.
Providing guidance and support to help you navigate the aged care system and make informed decisions about your care.
This helps ensure your care plan remains safe, individualised and up to date.
Support at Home Contributions
Under Support at Home, the government may ask you to contribute to the cost of your independence and everyday living services, based on your income and assets.
Contribution rates are set as a percentage of the price of each service. Below is a table of the contribution rates for each type of service and client

Income Assessment
Contribution percentages are set by Services Australia via an income and assets assessment. Completing an income and assets assessment isn’t mandatory. However, if you choose not to complete one, you can be asked to pay the maximum contribution rate. You can read more and start your application here.
